เรื่อง กำหนดการให้บริการกงสุลสัญจรประจำปี พ.ศ. 2553 (ค.ศ. 2010)

สถานกงสุลใหญ่ ณ นครชิคาโก กำหนดจะจัดให้บริการด้านกงสุลแก่ชุมชนไทยในโครงการกงสุลสัญจรประจำปี พ.ศ. 2553 ณ มลรัฐต่างๆ ดังต่อไปนี้

การให้บริการมีดังนี้

1. หนังสือเดินทาง
1.1 การทำหนังสือเดินทางอิเล็กทรอนิกส์ เอกสารที่ต้องนำมายื่นมี (1) หนังสือเดินทางเล่มปัจจุบัน (พร้อมสำเนา) (2) บัตรประจำตัวประชาชนไทย (พร้อมสำเนา) หรือสำเนาทะเบียนบ้านไทย (3) ค่าธรรมเนียม US$ 40.00 เป็นเงินสด หรือ Money Order หรือ Cashier’s Check สั่งจ่าย Royal Thai Consulate-General, Chicago การรับหนังสือเดินทางเล่มใหม่ใช้เวลาประมาณ 4 อาทิตย์
* เด็กเกิดใหม่ / ผู้เยาว์ ที่ยังไม่มีบัตรประชาชนไทยหรือยังไม่มีชื่ออยู่ในทะเบียนบ้านไทยให้ใช้ สูติบัตร (พร้อมสำเนา) หนังสือเดินทางของบิดาและมารดาและใบทะเบียนสมรส (พร้อมสำเนา) บิดาและมารดาต้องมาปรากฏตัว ลงชื่อยินยอมในการทำหนังสือเดินทาง
เพื่อความสะดวกและรวดเร็วในการทำ หนังสือเดินทางอิเล็กทรอนิกส์ ขอให้ผู้ประสงค์จะมารับบริการโปรดจัดส่งเอกสารที่เกี่ยวข้องให้สถานกงสุล ใหญ่ฯ ตรวจสอบความถูกต้องของข้อมูลล่วงหน้า คือ (1) สำเนาบัตรประจำตัวประชาชนไทยหรือสำเนาทะเบียนบ้านไทย (2) สำเนาหนังสือเดินทาง (3) คำร้องขอทำหนังสือเดินทางที่กรอกข้อมูลเรียบร้อยแล้ว (สามารถดาวน์โลด แบบคำร้อง e-passport 1 และ e-passport 2 จาก เว็บไซต์ได้) ส่งมาที่ Royal Thai Consulate-General, 700 N. Rush St., Chicago, IL 60611 เขียนมุมซองว่า “กงสุลสัญจร” ส่งเอกสารดังกล่าวได้ ณ บัดนี้เป็นต้นไป และติดต่อสถานกงสุลใหญ่ฯ เพื่อขอทราบเวลาที่จะรับบริการ ในวันที่คณะกงสุลสัญจรไปให้บริการโดยเพียงแต่นำหนังสือเดินทางและบัตรประจำ ตัวประชาชน/ทะเบียนบ้านมายืนยัน และชำระเงินค่าธรรมเนียมหนังสือเดินทาง สำหรับผู้มิได้ยื่นเอกสารล่วงหน้าจะได้รับบริการหลังจากที่ได้บริการให้แก่ กลุ่มล่วงหน้าเสร็จสิ้นแล้ว
1.2 การต่ออายุหนังสือเดินทางแบบเก่า ซึ่งยังไม่เคยต่ออายุในเล่มมาก่อน สามารถต่ออายุได้ถึงเพียงวันที่ 28 กรกฎาคม 2553 เท่านั้น จึงควรยื่นขอทำหนังสือเดินทางเล่มใหม่ ซึ่งเป็นหนังสือเดินทางอิเล็กทรอนิกส์ มีอายุ 5 ปี หากจำเป็นต้องขอต่ออายุหนังสือเดินทางแบบเก่า เอกสารที่ต้องนำมายื่นมี (1) หนังสือเดินทางเล่มปัจจุบัน (พร้อมสำเนา) (2) สำเนาบัตรประจำตัวประชาชนไทย หรือสำเนาทะเบียนบ้านไทย 2 ชุด (3) ค่าธรรมเนียม US$35.00 เป็นเงินสด หรือ Money Order หรือ Cashier’s Check สั่งจ่าย Royal Thai Consulate-General, Chicago ใช้เวลาประมาณ 1-2 อาทิตย์

2. การรับรองเอกสารต่างๆ เช่น รับรองลายมือชื่อ รับรองคำแปล และการเปลี่ยนนามสกุล (การเปลี่ยนนามสกุลต้องนำใบทะเบียนสมรสไปแปลก่อน แล้วจึงให้สถานกงสุลฯ รับรอง) ค่าธรรมเนียมรับรองเอกสาร US$15.00 เป็นเงินสด หรือ Money Order หรือ Cashier’s Check สั่งจ่าย Royal Thai Consulate-General, Chicago การรับเอกสารคืนใช้เวลาประมาณ 1 อาทิตย์
3. รับคำร้องขอออกสูติบัตร จดทะเบียนสมรส / ทะเบียนหย่า
4. ปรึกษาหารือเรื่องการกงสุล
